How to create a job application form with resume upload in Makeform

Create a job application form for free by adding contact fields and a File Upload block in the form editor. You can collect resumes, ask role-specific questions, and set an application deadline.

Is it free? Yes. Application fields and resume uploads are free within the Free plan's upload limits; team invitations require a paid plan.

Steps

  1. Describe your application form at makeform.ai/create. You do not need an account to start, and you can adjust the form with AI or edit it by hand.
  2. Click + (Add form element) and use Input blocks. Add Short Text Input for the applicant's name, Email Input, Phone Input, and Link Input for a LinkedIn profile or portfolio.
  3. Add File Upload for the resume. Its default label is “Upload your resume”, with the subtext “Accepted formats: PDF, DOC, DOCX.” Set Allowed File Types to the formats you want, such as pdf, doc, and docx, and set Maximum Files.
  4. Add Long Text Input for a cover letter or an explanation of why the applicant wants the role. Use choice questions for role, location, or work authorization. You can also add a Signature for a declaration.
  5. Use conditional logic for follow-up questions that depend on the selected role.
  6. For a deadline, go to Settings > Access and enable Close on scheduled date. Set Timezone, Date, and Time.
  7. Review applications in the Submissions table. Use Export submission PDF to export an individual application, or send applications to Slack, Google Sheets, or Notion through the free integrations.

Resume upload limits

On Free, each file can be up to 20 MB, and all files in one submission must total no more than 20 MB. You can add up to three upload fields per form and collect up to ten files per field.

Pro and Business support files up to 1 GB each, up to 2 GB per submission, and up to 100 upload fields per form.

Review applications with a team

Inviting teammates requires a paid plan. Pro supports Editor invitations; Business also supports Viewer and Admin roles. Individual submission PDF exports and the Slack, Google Sheets, and Notion integrations are free.
