How to create a form with AI from one sentence
Describe what you want to collect at https://www.makeform.ai/create, and Makeform builds a draft you can edit. You can start free without an account, then sign in to save and publish it.
Is it free? Yes. AI creation with the standard model, MF Mini, is free on every plan; Pro adds MF Pro, MF Next, and MF Ultra.
Steps
- Go to https://www.makeform.ai/create. If you are signed in, you can also click Create Form in the dashboard sidebar.
- In the box labeled What do you want to make?, type one sentence. For example: “A volunteer sign-up form with name, email, shift preference and a waiver checkbox.”

- Click Sign in to save & publish in the top bar. Sign in with Google or email; your draft and chat move into your account.
- Click Publish at the top right to get the link.
Write a useful prompt
Say who will fill in the form, what you need to collect, and any rules such as required fields, limits, or payment. Include what should happen after submission.
The AI can build multi-page forms, conditional logic, page routing, scoring, quizzes, calculations, several endings, redirects, and themes. You can also attach a PDF, Word file, or photo as a starting point.
