Steps

  1. Open https://www.makeform.ai/create or the chat box in your form editor. There is no separate importer.
  2. Click the + attach button, or drag your file into the chat.
  3. Type a request such as “Turn this into an online form. Keep the same questions and make name and email required.”
  4. Send your message to have the AI build the form.
  5. Check every question against your original document. Fix anything by clicking the question or asking the AI to change it.
  6. Sign in if you started without an account, then click Publish to publish the form.

Supported files and limits

You can attach PDFs, Word .docx files, and images in PNG, JPG/JPEG, WebP, or HEIC/HEIF format.

  • Attach up to 5 files per message.
  • Images and PDFs can be up to 1.5 MB each, with a 1.5 MB total limit for images and PDFs.
  • PDFs can have up to 5 pages.
  • Word .docx files can be up to 4 MB.

Use a photo or pasted questions

A photo of a paper form works. Make sure the text is sharp and the whole page is in frame, then check the generated questions against the photo.

You can also paste a list of questions as text and ask the AI to turn it into a form. For example, copy the questions from a Google Form or a document into the chat.
