Start with the name the classroom should use.
Collect the identifiers your teacher or program needs, but keep each label precise. A preferred or used name can guide daily conversation, while a legal name may be needed only where your school workflow calls for it. Avoid repeating information already available unless families are being asked to confirm that it is current.
