Identify the learner and the review window.
A rating without context is hard to interpret later. Record the student, reviewer, course or program, and exact evaluation period in structured fields. If the form serves multiple classes, use dropdowns to reduce spelling variations and make filtering cleaner.
- Student name or local identifier, grade or age group, and class or program.
- Reviewer name and role, evaluation type, and start and end dates.
- Unit, subject, activity, or target skill covered by this review.
