People and relationship
Identify both sides of the nomination.
Collect the nominee's name and work group in a controlled field when possible. Ask for the nominator's identity when follow-up may be needed, or deliberately omit it if the program accepts anonymous nominations. A relationship field can clarify whether the example comes from a peer, manager, direct report, or cross-functional partner without changing its merit.
- Nominee name, department, location, or shift.
- Nominator name and contact, when the program is identified.
- Working relationship for context, not automatic weighting.
