Introduction
Tell students what the survey is for.
The opening should say who is asking, what topic the survey covers, how long it should take, when it closes, and how the team plans to use the answers. State whether responses are anonymous, confidential, or connected to a student identity; those words are not interchangeable.
- Purpose, estimated completion time, and closing date.
- A plain-language description of who will review responses.
- An accurate statement about names, email addresses, and other identifiers.
