Explain the survey before the first question.
State who is running the survey, why responses are being collected, how long it should take, whether participation is optional, and how findings will be summarized. Distinguish anonymous collection from confidential handling: anonymous means the form does not collect identifying information, while confidential responses may still be visible to a limited reviewer.

- A precise anonymity or confidentiality statement that matches the setup.
- A contact or support route outside the response fields when appropriate.
