Purpose and consent
Explain why you are asking.
Open with the questionnaire's purpose, who will review responses, whether participation is voluntary, and how results will be used. Workplace respondents especially need to know whether managers see individual answers or only grouped themes. Do not promise anonymity if names, email addresses, unique links, or identifying metadata are collected.
- Name the sponsoring team and the decision the answers will inform.
- State whether responses are anonymous, confidential, or identified.
- Give an estimated completion scope and allow respondents to stop or skip where appropriate.
