Employee and work context
Identify the employee and the right internal contacts.
Start with structured work details rather than relying on a name alone. Employee ID, work email, department, location, and manager help HR distinguish similar names and direct operational questions. Avoid collecting unrelated personal identifiers just because the form allows another field.
- Employee name, work email, employee ID, and preferred contact details.
- Department, work location, role, and direct manager.
- A separate HR contact preference for communication during the absence.
