Open with a brief purpose statement, who reviews responses, whether participation is optional, and what happens next. Do not promise assistance the form itself cannot deliver. If a response is anonymous, say so only after confirming that the form does not collect names, email addresses, precise locations, or other identifying fields.
- Purpose, estimated effort, and intended use of responses.
- Clear option to skip nonessential questions or stop.
- Accurate expectation for follow-up, including when no direct reply is offered.
