Identify the right child and program group.
Begin with details that prevent staff from opening the wrong record. Preferred and legal names may serve different operational needs, while birth date, classroom, site, school, or camp session connects the response to the child's current placement. Include the household address only when your process needs it, and explain why each less-obvious field is requested.
- Preferred name, legal name, and birth date.
- Classroom, teacher, site, school, grade, or session.
- Home address and usual attendance schedule when relevant.
