Student context
Collect only what makes follow-up useful.
A name, grade, advisor, and optional student goal can help route the response. Explain why identifying fields are requested, and omit details that do not support the activity. For a classroom discussion, an anonymous version may be enough; for individual planning, the counselor needs a reliable way to reconnect the response to the student.
- Student name or local identifier when individual follow-up is planned.
- Grade, advisor, homeroom, or class period for practical routing.
- A short prompt asking what decision or question the student has today.
