# How to create a booking or appointment request form in Makeform

Collect booking requests for free with date and time fields or a slot list in the form editor. Makeform does not check availability or sync your calendar, so you confirm each request yourself.

**Is it free?** Yes. Booking request fields, slot limits, and owner email notifications are free on every plan.

## Steps

1. Describe the booking request form at [makeform.ai/create](https://www.makeform.ai/create), with no account needed to start, or start with a blank form. You can [adjust the form with AI](/help/guides/create-a-form-with-ai) or add fields by hand.
2. Click **+** (**Add form element**) and use **Input blocks** to add **Short Text Input** for a name, **Email Input**, and **Phone Input**.
3. Add a **Single Select** or **Multiple Choice · Select One** list of slots. Include both the date and time in each option, such as “Tue 3 June, 10\:00”. Set **Limit responses per option** to 1 so each slot accepts one response; a taken slot shows **Full**. Alternatively, add a **Date Picker** and **Time Input** for respondents to request a date and time.
4. Use **Add ending** to create a thank-you page. If you used a **Date Picker**, add **Add to Calendar** so respondents can add the chosen date (and time) to Google Calendar or download an `.ics` file. The block needs a **Date Picker** answer, so it does not work with a slot list alone.
5. Turn on **Self email notifications** so you receive each booking request. Owner notifications are free.
6. Publish the form. Confirm each request yourself; collecting a preferred date and time does not check your calendar's availability.

## Set date and time choices

A **Date Picker** supports minimum and maximum dates, such as today or a number of days ahead. You can also disable chosen weekdays.

In **Time Input**, use **Available times** to set the earliest and latest time and a minute step of 5, 10, 15, 30, or 60. Use **24-hour format** if that is how you want times displayed.

## Embed an existing booking page

If you already use Calendly or Cal.com, add **Booking page / Map** from **Layout**. This block embeds a Calendly or Cal.com booking page in the form; it can also embed a Google Map.

## FAQ

### Can I allow only one booking per slot?

Yes. Use a choice list with a date and time in each option, and set its response limit to 1. Full options display **Full**.

### Does a date picker check whether I am available?

No, Makeform does not check calendar availability. A date and time selection is a request that you confirm yourself.

### Can respondents add the booking to their calendar?

Yes, if the form has a **Date Picker**. Add **Add to Calendar** to an ending page; it uses the chosen date (and time from a **Time Input**) for Google Calendar or an `.ics` download. Without a date answer, the calendar buttons are hidden.

### Are confirmation emails free?

Owner email notifications are free. Respondent confirmation emails require Pro, which is also included in Business.
